Lifeways Malheur Intensive Children’s Admission Policy

Lifeways embraces a philosophy of proactive intervention that strives to facilitate the ability of a child and family to access services at lesser levels of acuity. Lifeways will ensure that children and families will have timely access to Intensive Community-Based Treatment and Support Services (ICTS) program.

 

Procedure


Admission to the ICTS program is a referral-based process. Referrals can come from any involved source, provided appropriate releases are signed when referrals come from persons or agencies that do not have legal custody of the child guardian sources.


The Family Care Coordinator (FCC) will assist the person or agency making the referral, as needed, with the collection of information for the referral packet and serve as point of contact for the submission of information. The FCC will respond to a referral within two working days of receiving the referral form.


A request for ICTS services will be considered authorized when:

  • ICTS Referral Form has been filled out and submitted to the FCC;
  • A consent for service has been signed by a custodial parent or other legally qualified person;
  • Appropriate authorizations for release of information necessary for the ICTS process have been signed by a custodial parent or other legally qualified person; and
  • A level of need determination has been completed.

A Child and Adolescent Service Intensity Instrument (CASII) will be one of the instruments used to make a “determination of need”. The CASII will not be used to exclude participation in the ICTS program. Services delivered within the ICTS will be tailored specifically to the goals and objectives designated within the CRT Service Plan. However, the CASII score will serve as one of the required criteria for a referral to psychiatric residential treatment services.
Upon receipt of the ICTS referral form, level of need determination, signed consent for services, and signed authorizations to release of information, the child and family will be admitted to the ICTS program and the child’s individualized Community Resource Team will be formed.
