COMMUNITY RESOURCE TEAM
A Lifeways Service for Children 0 to 18

 

What is it? A coordinated effort to avert crises.

 

Families who need this:

  • Are in crisis, or on the verge of crisis
  • Have many different supports that need to work together, or
  • Have no supports in place at all

What happens:

  • All interested parties are notified of a meeting date
  • A facilitated meeting takes place, ending with a specific plan
  • The need for further meetings is determined and scheduled

Who attends varies with each family, but can include:

  • School representatives
  • Medical representatives
  • Mental health counselors
  • Department of Human Services
  • Friends/relatives of the family

How you can help them get coordinated care:

  • Talk to the family and see if they want to participate in the process
  • Refer the family to Lifeways for an Assessment (at no cost)
  • Call Lifeways and talk to the Family Care Coordinator

 

What Program is best for your needs

Community Connections Network

 

This is a referral system for kids who have no services and/or no definition of need – no doctor, no therapist, etc. Usually the referrals are health-based, but not always. Referrals are made through the Health Department (889-7279, Cindy Ortiz) or Treasure Valley Pediatrics (889-2668, Gail Elliott). They can probably give you referral forms electronically. A meeting is convened of all possible resource representatives (psychologist, medical team, occupational therapists, etc.) to decide what services are needed and coordinate their implementation. Parents may or may not be involved. There are usually at least two follow up meetings for that child, to make sure everything is in place, as well as a review at the end of the year. This is a free service and anyone, including parents, can refer a child. Most referrals come from the schools.

 

Community Resource Team

 

This is a meeting designed for kids who are at risk of being hospitalized, put in foster care, or escalating to some other seriously negative outcome. Referrals are usually based on mental health or alcohol and drug issues, through the Family Care Coordinator at Lifeways (Debbie Thompson, 889-9167). Meetings are convened that must include parents (except in very specific circumstances), along with whoever else is material to the child’s success (school personnel, Child Welfare, Juvenile Department, therapists, doctor’s offices, extended family members, even neighbors, if desired). It is a very structured, one hour event that starts with pertinent facts posted on the meeting wall, and ends with a specific, time-sensitive plan for the upcoming month. The plans and progress are monitored on a monthly basis until the family is stable and/or goals are met. Anyone, including parents, can refer a child.
