Eligibility criteria for those whose condition originated before age 22 are established by Federal and State law. Eligible diagnoses may include autism spectrum disorder, cerebral palsy, epilepsy, Asperger’s syndrome, Tourette syndrome, Down syndrome, traumatic brain injury, fetal alcohol syndrome, Fragile X syndrome, Rett syndrome, Prader-Willi syndrome or other neurological conditions that can benefit from training or support.

Services are not based on income and may be in addition to other services received from Social Security. The application form on this site can be printed and submitted to the DD program for review. You can get more information by calling Lifeways at (541) 889-9167.
