The Westgate House Residential Treatment Home provides individualized services for up to five residents who are diagnosed with a serious mental illness.  Westgate House promotes mental, physical and emotional well-being by providing a supportive living environment. Westgate House is committed to working with residents to enhance the skills that allow them to identify and accomplish meaningful, personal goals, and to prepare them to re-integrate into the community of choice. Westgate House residential services are provided in collaboration with Lifeways’ Pendleton outpatient behavioral health service providers. Services are provided in a safe, respectful and welcoming environment.

The goal of all residents at Westgate House is to step down to more independent living within six to twelve months. Qualifying individuals must be medication compliant and stable, ready to take the lead in their own treatment and willing to fully participate in therapy, case management and house responsibilities.

These items are required to complete your referral request:

  • Westgate House Client Intake Form

  • Recent Substance Use Disorder (SUD) Assessment and ASAM (must have been completed within the last 30 days

  • Signed Release of Information (ROI)

These items are helpful to expedite the referral process and may be requested by the staff.

  • Current medication list

  • Most recent Mental Health Assessment

  • Most recent Psychiatric Assessment

  • Date of last Tuberculosis (TB) test

  • Date of last physical examination
