WRAP Systems of Care

Wraparound is a fidelity-based planning process that follows a series of steps to help children and their families realize their hopes and dreams. The Wraparound process is a team-based activity that helps groups of people involved in your family’s life work together toward a common goal. For some Wraparound teams, the goal is a team mission; for others, it is the family’s vision of how things will look in the future. The process is organized and delivered by someone who is trained to coordinate the team and facilitate the planning process. The team creates a plan that includes ways to ensure that children/youth and their families can experience success in their communities, homes, and schools.
Here are some key points about the Wraparound process:

How Wraparound works:

  • Anyone can make a referral to Wraparound with a youth and family’s agreement. The youth and family will be centered in a collaborative, strengths-based, and community-focused care planning process that uses strengths, needs, goals, and strategies to determine the best and individualized support for each youth and family.

Purpose and Goals:

  • A team-based care planning process.
  • Individualized and culturally and linguistically responsive care planning.
  • Inclusion of the youth and family’s friends, extended family, and other people in the youth and family’s life
  • Services and supports (both formal and informal) chosen by the youth and family.
  • Access to peer-delivered service providers for both the youth and family
  • Crisis and safety planning

Who Can Benefit:

  • Children/youth from 0-21 years of age
  • Children/youth involved in two or more systems including behavioral health, child welfare, special education, juvenile justice, and/or intellectual/developmental disability services.
  • Children/youth experiencing complex needs.
  • Children/youth exiting Secure Childrens Inpatient Program, Secure Adolescent Inpatient Program, Psychiatric Residential Treatment Services, or the Commercial Sexually Exploited Children’s residential program.
  • Children/youth who are Medicaid eligible.
