Child and Adolescent Services: Intensive Care Coordination

Intensive Care Coordination

A collaborative, comprehensive, team-based care coordination to help youth and families connect to resources and services to support behavioral health needs. Similar to Wraparound Care, the goal of Intensive Care Coordination is to connect youth and families to necessary services and supports within their community, as well as to work to coordinate care across multiple systems and agencies. ICC differs from Wraparound by not being a fidelity-based process and therefor being a more flexible option for youth and families feeling overwhelmed by Wraparound, as well as for youth and/or children over the age of 10 who are unable to attend or maintain throughout Wraparound meetings due to becoming activated or other circumstances.

Here are some key points about the ICC process:

How ICC Works

  • Anyone can make a referral to ICC with a youth and family’s agreement. An ICC team consisting of support and service providers identified by the youth and family will work together to assist in meeting needs and celebrating successes.

Purpose and Goals:

  • A more flexible, yet still intensive, planning process when compared to Wraparound
  • A team-based care planning process
  • Inclusion of the youth and family’s friends, extended family, and other people in the youth and family’s life
  • Access to peer-delivered service providers for both the youth and family

Who Can Benefit:

  • Youth and families who feel Wraparound is an overwhelming time-commitment.
  • Youth/children who do not have a stable placement and/or caregiver.
  • Youth/children over the age of 10 who are unable to maintain in or attend Wraparound meetings.
  • Youth and families who do not meet the 2 or more system involvement requirement for Wraparound
  • Children/youth who are Medicaid eligible.
